Participation Requirements

Participation Requirements for Ashland Athletics

Before any student can participate in tryouts/practice, the following requirements must be met:

  1. A parent/guardian must complete registration via FamilyID (link below).

  2. A student MUST turn in a copy of their most recent physical to the athletics office prior to the first day of practice.
Physicals are good for 13 months from the date of the exam. Students with expired physicals will not be permitted to participate in practices or games until a new physical is turned in.

  3. A student must submit either a user fee or the Athletic Fee Waiver to the Athletic office (mail or in person). Checks should be made payable to Ashland Public Schools. Students who participate in sports will not be given a uniform until the athletic fee is paid, a payment plan is set up, or you have submitted a waiver form for payment. Any questions please contact the Athletic Department for more information.

  4. Parent/guardian and the student-athlete are required to complete an online concussion course in the past 12 months. The free course can be competed online. See more information on concussions under Athletic Resources - Concussions (link below).

  5. All students and parents should be familiar with the Ashland High School Athletics Handbook. Download the Athletic Handbook.

  6. All students must be declared academically eligible (see Student-Athletic Handbook) and reflect behavior of a good school citizen.

PLEASE NOTE: This online registration form must be completed by a parent/guardian.
